Занятно завуалировали ошибку проектанта, особенно в свете

От одного из поврежденных топливных стержней при транспортировке отвалился фрагмент.

Во всех блоках твэлы вибрируют в потоке теплоносителя, но у проектировщика есть обычай (в свете падения "квалификции" очень правильное слово) отстраивать собственные частоты конструкционных элементов основного оборудования и топлива от несущих частот и их субгармоник.

Единственное, по тексту кажется идёт путаница между твэлом и кассетой.

говорит как раз о том, что в зону, похоже, пошли кассеты с отличающейся гидравликой и здесь аналогия с Вестом на Южке прямая. Теперь понимаете, почему я часто напоминаю, что осматривать надо не только кассеты Веста...

Со всем сказанным согласен, вот по поводу путаницы, сказать сложно. На сайте местного регулятора (Slovenian Nuclear Safety Administration) Андрей Стритар пишет в ответ всяким "зеленым человечкам" в документе от 22.10.2013 года (Odgovor na vprašanja o dogodkih med remontom NEK) следующее (переведено на английский, по ссылке оригинал):
